Oncology Patient Care Navigators

The role of the Navexio’s Oncology Patient Care Navigators is to accompany patients and their families in-person to their medical appointments and provide on-going support (virtual or in-person) for as long as it is requested throughout the patient’s cancer journey.

Navexio Oncology Nurse Navigators are Registered Nurses (RNs) and Pharmacists who have direct experience working with cancer patients and have an intimate understanding of working within and navigating our cancer care systems.

Ongoing support and care in between medical appointments is critical for optimal outcomes. Navexio’s Patient Care Navigators will work with patients and family members to help translate medical information and advocate where needed.

What you can expect to be provided by Patient Care Navigators: 

    • Ensure effective preparation for your appointment.

    • Meet you at your hospital/ clinic and join you at the appointment.

    • Listen and ask appropriate questions to the your oncologist and members of their clinical care team and take appointment notes.

    • Identify and help mitigate barriers and roadblocks in the your cancer journey.

    • Collaborate with clinicians, nurses, and allied health professionals, as needed.

    • Create summary report of the appointment and provide a tailored action plan, reminders, and next steps until your next appointment.

    • Offer guidance for symptom management.

    • Translate medical and clinical information so the you fully understand all relevant information and take time to answer all questions.

    • Communicate with family members and caregivers, as requested, to ensure everyone is aware of the critical information to help inform decision-making.

    • Direct you to other resources (organizations, etc), as needed or requested.

    • Provide referrals to the best cancer experts or allied health professionals that are most appropriate for your needs.

    • Arrange for home health services.

Process

The following is an overview of the steps involved in working with Navexio’s Oncology Patient Care Navigators for virtual and in-person services.

Virtual Consultations

  • A Navexio Oncology Patient Care Navigator will ensure effective preparation for the appointment by reviewing the Client’s medical records (if available).

  • A Navexio Patient Care Navigator will schedule an initial consultation to learn about the patient’s cancer journey, medical history, and discuss their needs and challenges. This will set the course for Navexio to develop a personalized oncology navigation and consultation roadmap.

  • The Navexio Oncology Patient Care Navigator will prepare a summary report of the appointment and provide a tailored action plan, reminders, and next steps until the next appointment.

  • Additional virtual consultations can be scheduled to support the unique needs of each Client.

    The Navexio Patient Care Navigator will be a trusted Partner in the Patient’s circle of care to provide ongoing support (virtually or in-person), such as:

    • Collaborate with clinicians, nurses, pharmacists, and allied health professionals, as needed.

    • Offer guidance for symptom management.

    • Translate medical and clinical information to fully understand all relevant information and take time to answer all questions.

    • Communicate with family members and caregivers, as requested, to ensure everyone is aware of the critical information to help inform decision-making.

    • Direction to other resources (organizations, etc), as needed or requested.

    • Provide referrals to the best cancer experts or allied health professionals.

    • Arrange for home health services.

In-Person Consultations

  • A Navexio Oncology Patient Care Navigator will ensure effective preparation for the appointment by reviewing the Client’s medical records (if available).

Navexio’s oncology Patient Care service plans offer flexible virtual support from licensed oncology nurse navigators and pharmacists providing expert guidance, appointment preparation, follow-up summaries, and personalized care coordination to empower clients throughout their cancer journey.

Monthly Plan: Navexio Nurse Navigation

  • Guaranteed access and scheduling for a pre-determined number of Oncology Patient Care Navigator consultations per month.
